I have worked for an organization1 from Nov 1999 - Jun 2004 and then Organization2 from July 2004 - Sep 2006. And from Oct 2006 i am working in Organization3 and still continuing to work there. My EPF has transferred from Organization1 and Organization2 to Organization3 and is reflecting in the EPFO Passbook. However my EPS does not reflect in the passbook. I had followed up with the EPFO for Organization 1 and Organization 2 and have got a copy of Annexure K with me, that shows the amount of EPS contributions .
I am now thinking of applying for Scheme Certificates. But Organization 1 is now merged and bought over by a new company and hence that legal entity does not exist. Same is the case with Organization 2. I have the following questions
1. Can i fill up Form 10C and take attestation from a Bank Manager and apply to the respective EPFO directly to issue me a scheme certificate. If so is there any other documents that i need to attach along with the Form 10C.?
2. Will the Annexure K help in any way to prove my Service period, as it had all the records of Pension Contribution
during my tenure of Organization 1 and Organization 2.?
3. If the EPFO returns the form stating it needs to be endorsed by previous employer, is there any other recourse?

In the transfer cases, the pension fund and the service will be simultaneously transferred to the new establishments. Please visit your present PF office accounts department and check the transfer details in person.
