If anybody has any idea where I can find a standard questionnaire for the same as I have to conduct a survey of the employees, then plz send a reply to this post.

i do not know how a standard questionaire developed by someone else could meet your project requirements. Because when questionaires are developed the person who develops it has certain objectives and identified variable to meet his /her requirements. And his/her questionnaire meet hi/her own objectives. WHere as your objective are most likely to be different from another questionnaire.
If you are to develop a questionnaire [please do collect artciled from a refereed journal and based on the literature review develop a questionnaire to meet your own project requirements.
