Leave Database Format In Excel - Xls Download

Hi,
Can anyone help me out to maintain the Leave Database format in Excel for the period from Jan 08 to Dec 08
The Leaves are given in following ways :
PL =15
CL =8
SL =8
The strenght of Employees is around 250.

Can anyone let me know as how PF is calculated what forms need to be filled in case of withdrawal,Transfer, Avail loan from PF etc.
