are u talking of a leave from or a leave card. be specific as there is a difference between the two. a leave form will only have a leave request from the employee.
a leave card should have the complete record of the employee for the current year and a column stataing the leave balance of the previous year. it should also have a column to calculate the leaves availed and balance leaves at the end of the current year.
