Duplicate e pehchan card of ESI - What is the process of reissuing the same in duplicate from employer's part?

Recently two employees among which one of them has lost her e pehchan card and another emoloyee's e pehchan card was mutilated by her child. What is the process of re issuing the same in duplicate from employer's part?

simply take a print out of e pahchan card from esic site and get it validated by employer seal and signature
