Hi, As per my Appointment Letter my appointment is on probation for six months. I have successfully completed the tenure of six months of probationary period and have also provided my review inputs to the HR team. However, its been quite some time I haven't yet heard on my probation confirmation.
Kindly guide me with sample reminder letter how to write to my company. regarding my confirmation that they confirm my appointment
& send me the confirmation letter.
Regards,
Rakesh

Why you need letter directly talk to them for the confirmation.

Dear Rakesh,
If the company provides confirmation letters to all as a practice, that shall be done.
If in a particular case it is missed out by HR, it might not be intentional.
And there is no harm in you reminding them to give the same. I do not think they will deny it, unless otherwise your case is dubious to the management.

What should be ideal tat for confirming after probation by hr

It should be immediately after completion of Probation, provided the operating department should be discussing with HR on the confirmation.
And in general HR gives that without any reminder after confirmation by operating department.
