While going to office I met with an accident, I don't claim earlier and having my treatment in private hospital.. now my injury is 3 months old... and I have to undergo a major operation .. is it possible under ESIC?? If yes than how?

you must go to the nearest ESI Hospital for further treatment, from there they could send you for specialty treatment if required. Otherwise you could submit your medical bills to your ESI local office for reimbursement.
Since you're covered under ESI Scheme you need to take leave through ESI only for all benefits.

Dear all,
The ESI scheme provides full coverage even while going to your work place or returning to your home but the accident report must have been filed within the specified time limit which is now quite late / delayed.
Even with specific reason of unawareness as the accident took place outside the establishment / factory, you can submit accident report through your employer which can be accepted by the local office with the approval of concerned regional office. In that event, your benefit rates for absence will be increased from general sickness to employment injury.
The second option is to attend the ESIC dispensary and get your self referred to designated hospital and you will get reimbursement of medical bills and wages on ordinary sickness benefits rates.
