we will be having our half yeraly appraisals in june......., and i also prepared the form for the same, but need to make few changes in order to make it more effective... so can some one suggest few points which must be present in any performance form.......
the points which i included are...........
1) personal details (employee name, designation, date of joining etc.......)
2) main roles and responsibilities
3) key performance areas
4) Job skills and competencies
5) soft skills and competencies
6) achievement in assessment period
7) strength nad weaknesses
8) training & development needs(areas of improvement)

Hi Chanchal,
You could include a self appraisal form, where in you can access employees views on the company, their own performance , their need . achievement.
This would help us retain an efficeint employee.With this your appraisal form will be perfect :D
