i am working on project for implementing HRIS for company which don't have such system. so can any one help for producing a questionnaire for knowing the requirement of the company from its employee

I am no HR expert. I am sure a study of HR Information Technology - How to Select Human Resources Information Technology will help you designing a questionnaire to ask staff views. Not knowing the size and nature of the organisation, it will be difficult even for experts to give you concrete answers.
