1. Make a list of the services you require, and what sort of budget you have in mind.

2. Put together a draft proposal.

2. Find out who the relevant person is at the hospital and send the proposal to them.

3. If they are interested, make an appointment to discuss the proposal and gain some insight into how they will provide the services - and at what cost.

3. If it is all favourable to both sides, get the legal people to draft a contract.

None of us work for your organisation, and we know nothing of how it works, what you do, and what YOU need.

There are NO standard documents. Every organisation is different and has different needs.

You have to do the research and write these documents for YOUR organisation. If you are unable to do it, then you may need to talk to your superiors about outsourcing this project to a contractor.

I would suggest talking to the hospital first and seeing if they are interested. If they express interest, then you have to work out what to do. They may be able to assist you with documentation, but you will need your legal people involved with this as well.
