Dear Vasu,
You have asked the format for the show cause notice. However, show cause notice is not a form as such wherein one just fills the blanks. It is issued to the employee who has conducted some misconduct.
In the show cause notice, we need to include how did you discover the misconduct, what was the wrongdoing of the employee, where he did and when he did.
Therefore, please give us the complete background of the case. Otherwise members of this forum, who are outsiders, cannot determine the nature of the misconduct and it will not be possible to provide you draft of the show cause notice.

You have need the format or ready draft for the show cause notice. However, show cause notice and other documents or disciplinary letter like warning letter and charge sheet is not a ready-made drafts, as such wherein one just prepare the fill in the blanks or fields.
In the show cause notice, we need to include the actual facts which was committed by employee. Such type of notices are totally depends on particular misconduct like Absenteeism, interfere with safety guard, negligence on duty, theft, abusive language towards superior, slowing down work, under influence of liquor while on duty and sexual harassment etc. Hence you should collect factual information and investigation of each and every facts which are connected or related to such misconduct.
Be conscious, when you will drafting any notice. It must be specific and must set-out the necessary particulars. It will helpful for further Enquiry and Adjudication.
