Our empolyee died while in service on 2009 his spouse want claim the epf 2015 and remember me about the claim she apply for that this was come into enforcement to local epf office the officer enforced and he asked records of attendance and salary but salary register not available and attendance register available but in attendance we give attendance to him about one month lack of awareness and salary in epf statements the above issues the officer asked reasons with affidavit so please how to make the affidavit satisfying the above issues to EPF officer

Nothing is required to be shown to EPF dept or concerned officer but she is eligible for pension and ELDI benefits through EPF and she will get pension and other benefits from date of death of her husband.you might be having record of his joining provident fund and date of last contribution .
That is only required .you may be having his PF number and PF department will be having all details.if you have any problem please contact me ,i will help you out.
