Hi Muskan,
You mean to say efficiency report? B coz productivity report is to be prepared by production department.
Efficiency report can be prepared by HR dept. based on criteria set by the management. Efficiency can be based on Piece rate/ Hourly rate etc.
You pl. clarify your clarify ur query then only the members are able to shower some inputs.

The report needs to clarify the performance of an individual in a particular department for a specific period. In other words mapping an individual performance
