could you let us know what is departmentalization in our own understanding and knowledge?
What departmental change you required, please explain it further so that member can understand your query well and give you responses accordingly
Second, change in department depending on the work functions and related things.

You would need to issue a letter stating that the employee name performance was satisfactory in the probation period therefore your dept. is changed to abc dept. and designation will be xyz.
attached the new salary structure and get signed from employee as well as authority after that you can change the dept.
