I do not know by which mode you are doing your course and what the course requirements are. First a bit of advice and then a criticism of your questionnaire. To improve the response rate to your questionnaire, it is better to use a web-based survey. Now, you expect people to download your questionnaire, fill it, save the document and repost it here. This is time consuming and also everybody will be able to see the responses. Thus there is no confidentiality.
Now some critical comments. As an ex MBA tutor and a researcher, I find that without a covering letter explaining the objectives of your survey and a note how the information will be used (for example, you seek the name of the HR person and the organisation), many will not answere the questionnaire. The questionnaire seems to be very basic for even an undergraduate project as it contains only bipolar and simple openended questions. An MBA student is supposed to exhibit a knowledge of applying statistical tests, etc.
Sorry for the criticisms; but these will help other students to design their questionnaires, if it is too late for you to change.
