Training Card - Xls Download

Hi friends,
I am workin as Sr. Officer - P&A with Hindustan Composites Ltd., Paithan, Aurangabad (MS).
I have to prepare Traning Card for my employees to whom given training and to be given in future.
Can you suggest me how to prepare training card or post any format of Traning card?
