Hi Anil,
Please check the Govt Gazatte for info on that.
The salary slip format depends on the industry, the state govt, etc.
For example, if you check the booklet for
Catering Establishments in Tamil Nadu you will find the prescribed format for Hotel and Restaurants in Tamil Nadu.
