Dear Lakshmi PV,
To get employee pehchan card, you have to fill up the online application in your ESI account. Having completed the registration, you can download a Temporary ID Card along with Medical Acceptance Card. Employer has to affix the family photo of the employee on the required space and to stamp the same with the official stamp. Then the Employee shall produce the Temporary ID Card to the nearby ESI branch and have his and family members photo and biometric signatures taken. After a couple of days, the pehchan card will be sent to the company which is in turn to be handed over to the employee.
