I need your help in filling up a reference check form for my friend for his job.
What should be the answers for the following questions:
1.In what capacity have you known him? Clarify.
2. Kindly give us your candid opinion about him in respect of the following:-
i. Ability and Performance
ii. Conduct and Integrity
iii. Character and Antecedents
3. Any other point which you man consider relevant in this context.
Pls guys give me some answers i need it urgently.

For the first question, They asked in what way you are related to him that is technical (Direct)or non technical(administration), whether he used to report to you or you worked with him in different department
for the second question you should mention his quality in the way of percentage out of 100 or out of 10
1.e Mention , how he will perform in his job, how he use to initiate new processes (90% or 8 out of 10)
Specify how he will furnish his ability for trust worthy
Specify how he will behave and co-ordinate with other employees
for the 3rd question you can specify his other good conducts relevant or irrelevant to his job(participation in events / organizing any special occasions)
