I am working on the Kirkpatrick's model for quite a while now. Have got to read some very interesting articles through all the links posted here. I am trying to find out the importance of using a Control Group for analysis of Level 3 effectiveness of the model.
Also, can anyone help me with a questionnaire for level 3-4 analysis?

For Kalpana: - The questionnaire that you have given is not suitable to measure the Level II change. To measure the Level II change you must conduct the post-training test. Mere asking the participants whether they have implemented the learning or not is sufficient. It goes without saying that nobody will say 'no'. Level III change is measured through change in various ratios. Level IV change is measured by measuring business impact because of these change in ratios.
