Dear Rex Martin,
Go to his previous employer with form 19 and fill the form with the datas collecting from there. Get endorced the form 19 by that employer and submit the RPFC (belongs to the previous employer). This is the procedure to withdraw PF amount.
If you want to transfer the PF, fill form 13 and send it to the same RPFC through the present employer. This will be adviseable as the EPF and EPS can be transferred by a single process.

Hi
Fill up the form 19, get verified the form 19 by the employer or any area MLA, SDM, Parsad, Gaztted officer etc and submit the RPFC (belongs to the previous employer). This is the procedure to withdraw PF amount.
Or, fill up the form 13 for transfer of PF to present A/c No and send it to the same RPFC through the present employer.

Dear Friend,
To my knowledge and personal experience, If employee want to withdraw his P.F accumulations of his ex -employer after three years he has to submit affidavit . This affidavit format can be obtained from P.F office. This Affidavit has to submit along with form 19 and 10C.

Hi,
I am still confused, can anyone tell me that if I withdraw PF amount after completion of 3 years continue service. In that case can I withdraw Family Pension Scheme (FPS) amount too. I heard that in this case I will be entitled to withdraw the PF amount only (12%+3.67%=15.67%). Please clarify/help, I am afraid to heard that I have to complete 10 years continue contribution in PF Account to withdraw my FPS amount too otherwise the same shall be forfeited and I will get only 15.67% amount of 24%.

Dear Vijay,
you can claim your full amount of pf (12% + 3.67%) and pension (8.33%).
for that you have to submit form 19 & 10C, for claiming pension amount you have to complete minimum six month of service. you can claim total 24% amount because you are completed 3 year service, If your service is crossed above 10 year, you will be eligible for monthly pension after crossing your 58 year of age.
hope it may clear for you.
