Dear Priyanka and Viji,
Don't waste your time in making appointment letters for drivers.
As per Karnataka Shops and Establishment Rules, we are supposed to issue Form 10. This form 10 is appointment order by the employer. I use to issue the form 1o for lower level. Further I use to attach standard terms and conditions of employment.
You may find out requisite form from the shops and establishment act or factory of your state.
