Generally PMS method folow with

• Self Appraisal
• Appraisal by superiors - Measure the KRAs for the passed year
• Meeting b/w the appraiser & Appraisee
• Defining KRA for the next year
• Defin goals & Objectives

just i have given a broad idea. Try to prepare you own template as per your requirements.
