The format sent by Mr Rajesh is really suited all the need to appraise an executive or manager. Still there may be some variations depending on the type of ur industry e.g. service/ manuf./ IT/etc...., kind of job the executive handle e.g. office job means independent/ No sub ordinates/ stationed job/ etc. Some weightage can be given on the basis of traits reqd i.e. called competency based appraisals.
