Hello friends I’m doing my MBA.now i’m insearch of sample questionnaire for my project in performance appraisal in manufacturing sector.can anyone help me or sent me any questionnaire

Dear Vineet,
Your forms are really practical. One thing still I did not understand is, in PMS format, what I supposed put in "Measurement Criteria" and in "Weightage"?
Please guide me.
