You can update correct bank a/c & name with your unified member portal itself. and employer's also can update your correct bank a/c and name with your KYC.
You should submit your correct bank details to your employer.........

When there is wrong KYC details were entered then we need to enter correct KYC details once again and employer needs to approve those new KYC details again with their digital signature. You can always correct and update your bank account details in PF whenever you required even though they were already approved. To change your bank a/c details again enter your new bank account details in UAN portal and tell your employer to approve them.
Once they approve them automatically your old bank a/c details will become inactive and new bank a/c details become active.
