You can submit Form 19 with your ex-employers to get the PF amount of your share i.e., only 12%. Employers contribution cannot be paid unless you have continuous service for 6 months.

Dear All,
1. Every employer shall require to attest the claim application of Para 69 within Five days of its receipt U/P. 72 ( 5 ) (a).
2. If you are unable to get attestation in claim application for any reason whatsoever, you may forward it to the Commissioner or any authorized officer authorized by him in this behalf, The Commissioner or any officer authorized by him in this behalf, may forward such application to the employer and the employer shall be required to return it within five days of its receipt U/P. 72 ( 5 ) ( d ).

Yes, You can submit P F withdrawal application form No. 19 only to get your P. F. amount U/P. 69 ( 1 ). Application shall be submitted after completing a continuous period of not less than two months from date of exist from employment.
You are not entitled to receive withdrawal benefit under para 14 of Employees' Pension Scheme, 1995. Actual service should be at least more than six month for new entrant U/P. 9 ( a ) of Employees' Pension Scheme, 1995. Whereas your actual service is less than six month.
You are only entitled to receive Employee's share. Employer's share ( 12 % + 3.67 % ) and its interest thereon.
