Let me know where you are working and what act applies to your firm, for e.g. Shops & Commercial Estt. or Industry/Factory, Educational Institutions etc.
There are certain prescribed format for maintaining the Leave Register & Wage Register under the applicable act. Even you can see those format if you have copy of Act applicable to your establishment.
Reg.application for grant of leave, you can design your own format of 'leave application' suitable to you.
