Respective senior, Can somebody share a performa of joint declaration (employee and employer) allowing members of EPS95 the benefit of the actual salary in the pension fund exceeding wages limit of either Rs.5000/- or Rs.6500 pm from the effective date as per Hon\'ble Supreme court\'s order.
