Need Help Regarding Salary Slip - Doc Download

Respected seniors,
i am working small it firm since 1.10years so in our organization their is no practice of issuing salary slip as i am getting 12500rs by cash so in terms of growth now i want to leave this company,for that i need salary slip so i created salary slip by own so please check is any changes need to be so i am thinking to take signature and co-stamp of employer .Thank you.here its attached below.

Hi Lakshmikant,
PFA Salary slip format which does not need the employer's signature. I use this in my organisation.
You just have to enter gross salary and what deductions have been done. and total working days
