dear all
one of our employee left one year back after the 11 months of service, their after he expired with out withdrawal of his provident fund.
can anyone tell me what is the procedure to withdraw his PF. What documents and information does his nominee needed to withdraw the PF.

Dear Kulkarni
Following Forms to be submitted :-
Form – 20 : In the event of death of member, this form is to be used by a nominee / family member to claim the member's Provident Fund accumulation along with Death Certificate of Member.
Form – 10D :To be submitted in duplicate by the
- widow/widower or
- orphan or
- nominee as the case may be for claiming pension along with
Death Certificate, 3 passport copy of photographs separately for Widow and upto two children (below 25 years)
Form – 5 (I.F.):To be preferred by the person eligible to receive the Employees' Provident Fund dues of the deceased member who died while in services along with Death Certificate.
