Thank You, Your attached sheet is useful but i want to sheet with employee name, Each equipment used by each employee in month , this sheet keep with supervisor and filled by them.

Ohhpp !! i think have it in my office LT will check it on Saturday evening forward to you i have 2 days leave so.. just wait for 2 days i hope some of our members will help you. . .
Note: It's very easy to make it in XL sheet i.e; Name/ equipment name/ designation/ Required PPE/ Issued on / Supervisor signature & Employee signature thats it you can make it. lt's try bro. . .
