What type of form you required, under factories Act there are various forms starting from the opening of factories to till closer and there will litter variance in form number it varies to State to State and for which state you need form
nowa days these are available in repsective state Labour department website.

This is to inform you that The Factories Act, 1948 does not provide us forms. It is the Rules which provide us forms & therefore first examine which state /central Rules are applicable in your case & then go through those Factory Rules to have different forms for compliance.
