I am seeking guidance to evaluate an Employee's performance based on an Appraisal Form. I have framed this Appraisal Form for my Organization, in which there is a provision to mark an Employee's behavior on the Rating Scale 1 to 5. There are also columns where Employees shall write what they feel about their achievements in relation to the targets set initially. Please help me with the procedure of evaluating such form.
